Data Use Agreement - Details

To access CCWHC data, you must first read and sign the following agreement.

You must then fax or mail (electronic submissions will not be accepted) a signed copy of page four of this agreement to the relevant CCWHC site for authorization (see list below). Once we have recieved and verified your request you will receive an email regarding your account. Please note; illegible or incomplete submissions may take longer to verify and process. If you are unsure of whom you need to contact or what kind of access you need, feel free to contact the CCWHC IT Department at helpdesk@ccwhc.ca.
